Obverse: Crowned bust of Hormizd right, wearing earring. Eagle and korymbos in crown.
Obverse legend: Pahlavi (Middle Persian) inscription in margin surrounding.
Reverse: Fire altar with attendants flanking and garland, bust of Ahuramazda in flames.
Reverse legend: Pahlavi (Middle Persian) in left and right margin.
