In Boris’s apartments, his daughter mourns the death of her fiancé. Boris comforts her tenderly, talks intimately with his son about inheriting the throne and its responsibilities, then reflects to himself on the crime that gained him the throne and the fears that torment him.

The setting provides the backdrop for a series of events, starting with the introduction of Boris’s children, Ksenia and Fedor, and Boris’s monologue about his power and his crime. When Shuiskii enters and delivers the news of the pretender’s existence, Boris can’t take the tension anymore and hallucinates about the rising of the dead tsarevich.
